Community Connections Program (CCP)

Community Connections Program is a short-term linkage program that offers engagement, practical assistance and referral to appropriate services for people who are:
  • Confronting homelessness and/or who are living in unsafe or low-cost accommodation.
  • AND Socially isolated and are not linked to services.
Some of the services we can help you link into are:
  • Health Services
  • Housing Services
  • Community Support Services

Housing Support for the Aged

If you are 50 years of age or over and:

  • Currently live in unsafe or low cost housing AND/OR or at risk of homelessness.
  • AND Facing a wide range of serious difficulties in your life for which you need on-going support.
Housing Support for the Aged assists people who are aged 50 years or more who are confronting homelessness and who require on-going support to access they services that may help them.

SRS Mental Health Project ( Supported Residential Services & CCP)

  • The aim is to create links for people with psychiatric disability, living in Support accommodation to access local community supports and main stream services.
  • Community Connection Program aim to link other residents on the pension level in Support Accommodation with casual assist for Health Services and Community Support Services.
